COMMUNITY OUTREACH ASSISTANT

NOTICE TO ALL APPLICANTS: This

SUMMARY OF POSITION:
The primary function of this part-time
position is to assist Community Outreach Workers with client coordination and
case management services for economically disadvantaged families and
individuals. Such services include identification of community programs and
resources and referrals to public and private agencies and service providers.
This position requires the ability to work flexible hours and reports directly
to the Deputy Director of Health.
MINIMUM QUALIFICATIONS: Graduation from an accredited college or university with a Bachelor’s degree in Human Services or related
field preferred and two (2) years experience doing comparable work.
